What are the responsibilities and job description for the Physical Therapist position at LANDMARK HOME HEALTH CARE?
GENERAL SUMMARY:
Provides evaluation information to assist the attending physician with the diagnosis and treatment of patients with
musculoskeletal, neurological, orthopedic or cardiopulmonary disorders. Physical therapy services are provided under the
direction of the physician, with the participation of other health team members.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Assists the physician in evaluating and assessing the level of function of the patient and in developing a Plan of
Care.
2. Interprets findings to the physician, patient, family and the professional nurse.
3. Assists in establishing rehabilitative plans and goals to reach the patients optimum functional level.
4. Provides comprehensive physical therapy treatment utilizing rehabilitation procedures to improve the patient's level of
functioning.
5. Advises and consults with patient's family and the professional nurse in implementing and evaluating the Plan
of Care.
6. Revises the treatment plan as indicated, upgrades program based on patient's progress and participates in discharge
planning.
7. Provides supportive care to the patient, family and significant others and assists them to accept and adjust to
realistic plans and goals based on the patient's physical limitations.
8. Assists with the selection and/or modification of equipment, appliances and physical set up of the home to meet the
patient's needs.
9. Recognizes the teaching needs of the patient, family and significant others. Develops a teaching plan to meet those
needs.
10. Initiates interdisciplinary communication to assure the effective implementation of comprehensive treatment.
Participates in periodic patient evaluation conferences.
11. Prepares clinical progress notes and completes required documentation promptly, accurately, and concisely in
accordance with Agency Policy, Medicare and other third party reimbursement regulations.
12. Has access to electronic or manual protected health information by scope and responsibility. Will be
required to adhere to the privacy practices as detailed in the Notice of Privacy Practices, privacy policies and
procedures.
13. Maintains Professional License by completing the State mandated Child Abuse CEU’s.
Qualifications:
OTHER DUTIES AND RESPONSIBILITIES:
1. Maintains patient confidentiality.
2. Practices physical therapy within the guidelines of the Physical Therapy Practice Act and in accordance with the
patient's Plan of Care.
3. Identifies the need for medical intervention and for interdisciplinary referrals and acts accordingly to see that
those needs are met.
4. Participates in Patient Case Conferences and Quarterly Chart Audit Reviews as needed.
5. Assumes other duties as assigned by management.

KNOWLEDGE, EXPERIENCE, LICENSURE REQUIRED:
1. Graduate of an accredited School of Physical Therapy
2. Currently licensed to practice as a Physical Therapist in Pennsylvania.
3. At least one (1) year of experience as a Physical Therapist.
4. Comprehensive knowledge of current physical therapy theory, practices and procedures, and the ability to adapt
them to home situations.
5. Knowledge of current home health insurance reimbursement regulations, especially Medicare.
6. Currently licensed to operate a motor vehicle in Pennsylvania and necessary insurance coverage as mandated by
the state.
7. Maintains current CPR certification.
PHYSICAL JOB DEMANDS:
1. Ability to physically move, lift or shift 100 lbs.
2. Ability to stand, walk, climb and descend stairs, reach above objects involved in patient care.
3. Able to safely operate a motor vehicle.
WORKING CONDITIONS:
1. Normal working hours are Monday through Friday, 8:00 a.m. to 5:00 p.m. or as schedule dictates.
2. Maintains a safe driving record and a vehicle in dependable working order.
3. Participates in rotating weekend assignment rotation.
