What are the responsibilities and job description for the LPN position at LANDMARK HOME HEALTH CARE?
GENERAL SUMMARY
Provides nursing services in accordance with the Nurse Practice Act, on an intermittent basis to patients in their place of residence in accordance with a signed Plan of Care. The Licensed Practical Nurse (LPN) is assigned to patients by the professional nurse and functions under his/her direction.
DUTIES AND RESPONSIBILITIES
1. Provides nursing care following the established Plan of Care. Reports clinical findings to primary
nurse on a daily basis.
2. Evaluates the patient's progress. Informs the physician and other responsible personnel of significant
changes in the patient's condition. Discusses with patient/caregiver any changes in the patient's care
plan.
3. Provides instruction to patient/caregiver regarding disease process, treatments and goals.
4. Completes patient documentation within the established time frame assuring information is
appropriate, meets all third-party reimbursement requirements.
5. Participates in interdisciplinary case conference when necessary.
6. Assists the primary nurse in updating the nursing care plan at regular intervals and as necessary.
7. Provides a high level of customer service to internal and external customers.
8. Adheres to established company standards, policies and procedures.
9. Adheres to established Infection Control policies and procedures.
10. Participates in agency-wide Performance Improvement Program.
11. Documents patient care in a manner that is both outcome directed and addresses each individual
intervention included in the patient plan of care.
12. Insures that documentation indicates that interventions, outcomes, and goals included in the plan of
care have either been met or modified. If modified, documentation must reflect reasons.
13. Insures that all documentation meets "minimum requirements for charting" as identified by
Landmark Home Health Care Services policy.
14. Completes all visit documentation within 24 hours from the time service is provided.
15. May have access to electronic or manual protected health information by scope and responsibility.
Will be required to adhere to the privacy practices as detailed in the Notice of Privacy Practices,
privacy policies and procedures
OTHER DUTIES AND RESPONSIBILITIES
1. Attends conferences, in-services, seminars and meetings to develop professional expertise.
2. Performs other responsibilities as assigned by management.

KNOWLEDGE, EXPERIENCE AND LICENSURE/REGISTRATION/CERTIFICATION REQUIRED
1. Graduate from an accredited School of Practical Nursing.
2. Current license through examination by the State Board of Nursing Examiners of Pennsylvania.
3. Minimum of two (2) year medical/surgical nursing experience preferred.
4. Current CPR certification.
5. Practical Nursing knowledge, preparation, experience, skill and understanding to perform required
tasks.
PHYSICAL JOB DEMANDS
- Ability to move, lift or shift 100 pounds.
- Standing, walking, climbing and descending stairs
- Reaching over head and lifting
- Carrying/moving, stooping and bending
- Operation of a motor vehicle
WORKING CONDITIONS
Monday through Friday, 8:00 AM to 4:30 PM. Driving involved. Visits patients in their homes who may have a vast variety of socioeconomic backgrounds.
