What are the responsibilities and job description for the Clinical Dietitian position at Landmark Healthcare?
Landmark Infusion is seeking a clinical dietitian to join our team! This role offers the opportunity to work closely with a multidisciplinary care team to deliver high-quality, personalized nutrition support to patients managing complex medical needs at home.
Candidates are required to live in Dallas-Fort Worth area and commute to our office in Irving, Texas. We are looking for someone who has great time management and strong communication with other staff. We are looking for RD with a CNSC certification preferred. Candidates with TPN experience is strongly preferred.
Initial Dietitian Assessments
· TPN pharmacist will reach out to dietitian when a new TPN patient is to be admitted to Landmark service
· Dietitian will review clinical documentation, perform comprehensive nutrition assessment by phone, conduct medical nutrition therapy and make individualized recommendations for new TPN patients within 4 days after discharge from hospital with Landmark Infusion
· Initial assessment will be documented in medical record no later than 3 days of initial assessment, TPN Dietitian Assessments will be paid out in ADP Payroll once patient is discharged from hospital on TPN supplies with Landmark Infusion
· Assessment and Documentation timelines dependent on Registered Dietitian’s availability (i.e. vacation, scheduled days off, weekends included)
Coordination of Care
· Communicate/work daily with Landmark staff
· Discuss lab results, weight changes, weaning protocols, diet advancement, etc.
· Provide verbal recommendations to pharmacists
· Communicate information on new nutritional guidelines and best practices
· Weekly communications regarding Landmark Patients and Landmark Nutritional program between TPN pharmacist and TPN dietitian to be summarized by TPN dietitian in an email and sent to working group (email contents and group to be defined later but will reference at a minimum which Patients/topics were discussed.)
· Other tasks delegated by Landmark staff include creating monthly reports for physicians, documenting weight charts, speaking with referrals, etc.
Additional Patient Encounters
· Follow up with patients as needed when directed by TPN pharmacist
· Dietitian will review clinical documentation as needed, perform follow-up nutrition assessment by phone, conduct medical nutrition therapy and make individualized recommendations for existing TPN patients
· Follow-up note will be documented in medical record within 3 business days of consult from TPN pharmacist, or as otherwise desired by TPN team
· Dietitian would be available to speak with referral sources including dietitians, physicians, mid-level practitioners, etc. as needed if advised by TPN pharmacist to obtain throughout follow-up information
JOB REQUIREMENTS:
- Bachelor's degree, Master's preferred minimum in Nutrition, Dietetics, Food Science, or related field
- Must be a Registered Dietitian (RD/RDN) and licensed in Texas
- Completion of an ACEND-accredited dietetic internship
- Board Certified Specialist in Nutrition Support (CNSC)
- 1–3 years of clinical dietetics experience minimum
- Strong communication skills for phone-based or telehealth patient counseling (much home infusion dietitian work is remote/phone-based)
- Ability to work independently and manage a caseload
- Detail-oriented documentation for compliance and audit purposes
- Comfortable with EMR/EHR systems and infusion-specific software
