What are the responsibilities and job description for the Graceful Transitions Associate position at LANDIS COMMUNITIES?
Coordinates and completes downsizing, organization of the home, and décor tasks as part of transitions Services to those at Landis Homes, its affiliates, and the greater community. Works flexible hours to provide services when needed.
- Assists in the development of this service and potential future lines of services in conjunction with coordinator.
- Meets with clients and potential clients to review available services.
- Coordinates services for clients' needs such as scheduling and arranging manpower and outside services.
- Assists in the marketing of these services to internal and external customers and referral sources in conjunction with coordinator.
- Works closely with clients and/or families during transition to provide support for organizing and downsizing tasks such as identifying items to move, sell, donate, and discard.
- Assists with decorating and storage of decorations when not in use.
- Assists with outside maintenance such as flowerbeds and décor.
- Assists with monthly/annual reports as requested related to this service.
- Performs with a high degree of professionalism and has good time management skills.
- Relates well with residents, clients, family members, staff and the community.
- Must be at least 18 years of age.
- Must have high school diploma or GED.
- Minimum of Associate's degree in Human Services field, or equivalent experience, preferred.
- Experience coordinating projects and/or people required.
- Possesses strong customer service orientation and is a team player.
- Able to work independently, well-organized, and can multi-task.
- Demonstrates a pleasant personality with flexibility in meeting with clients as requested.
