Home Health LMSW Required Experience
- Minimum 1 year of skilled Home Health experience.
- OASIS experience required.
- Strong care coordination, communication, and patient advocacy skills.
- Active Licensed Medical Social Worker (LMSW) license (or ability to obtain prior to start; pending license accepted).
- Basic Life Support (BLS/BCLS) certification.
Traveler must have a reliable personal vehicle for daily home visits.
Daily travel is efficiently routed, with occasional travel up to approximately 60 miles (60–90 minutes total drive time).
Facility requires completion of the primary COVID-19 vaccination series plus one booster.
