Travel RN Case Manager - $3,123 per week

LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Burlingame, California.

Shift:

• Variable 5x8-hour schedule between 07:00–15:30 (day shift) and 15:00–23:30 (mid shift)
• Every other weekend rotation required
• Must be flexible for floating across inpatient units based on facility needs
• Schedule may vary depending on census and staffing demands

Responsibilities:

• Coordinate inpatient discharge planning from admission through transition to post-acute care
• Collaborate with physicians, nurses, social work, and interdisciplinary teams to support safe patient throughput
• Manage complex discharge cases, including SNF placement, home health, DME, and community resources
• Support ED and inpatient units with case management needs to reduce delays and improve flow
• Ensure compliance with California payer rules, insurance requirements, and county regulations
• Work independently in a fast-paced acute care environment with minimal supervision
• Document all case management activities in Epic (preferred EMR) and maintain accurate care coordination records

Requirements:

Active RN license
BLS (AHA)
Minimum 2 years inpatient case management / acute care discharge planning experience
Strong inpatient ED case management background (not UM-focused)
CCM/ACM preferred
Epic experience preferred
