Home Health Physical Therapist opportunity providing care to adult and geriatric patients in the community setting. Previous Home Health experience is not required, making this a great opportunity for therapists looking to transition into home care.

• Conduct initial and ongoing physical therapy assessments
• Perform head-to-toe evaluations and vital sign monitoring
• Develop and implement individualized treatment plans
• Provide:
  • Balance training
  • Strengthening exercises
  • Gait training
  • Transfer training
  • Stair training
  • Fall prevention education
• Assess durable medical equipment needs
• Collaborate with interdisciplinary care teams
• Support safe discharge to community living

• Approximately 6 patient visits per day
• Occasional weekend coverage as needed
• No call requirement
