Home Health Physical Therapist opportunity providing care to adult and geriatric patients in the community setting. Previous Home Health experience is not required, making this a great opportunity for therapists looking to transition into home care.
Patient Population

- Conduct initial and ongoing physical therapy assessments
- Perform head-to-toe evaluations and vital sign monitoring
- Develop and implement individualized treatment plans
- Provide:
- Balance training
- Strengthening exercises
- Gait training
- Transfer training
- Stair training
- Fall prevention education
- Assess durable medical equipment needs
- Collaborate with interdisciplinary care teams
- Support safe discharge to community living
- Approximately 6 patient visits per day
- Occasional weekend coverage as needed
