Job Summary:
We are seeking a dedicated and compassionate Occupational Therapist for a Home Health setting. The ideal candidate will provide high-quality, patient-centered care to individuals in their homes, helping them regain independence and improve daily living skills.
- Assess patients’ functional abilities and develop individualized treatment plans
- Provide therapeutic interventions to improve mobility, strength, and daily living activities
- Educate patients and caregivers on treatment plans and home exercise programs
- Monitor and document patient progress and adjust care plans as needed
- Collaborate with physicians, nurses, and other healthcare professionals
- Ensure compliance with all regulatory and documentation standards
- Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy (OTD) – Required
- Active Occupational Therapy License in applicable state(s) – Required
- AHA BLS Certification – Required
- Valid Driver’s License (DL) – Required
- Must be at least 21 years of age or older
- Home Health experience preferred, but not required
