Candidate Requirements
- Minimum 2 years acute hospital Case Management experience
- Must have recent CM experience within last 2 years (HARDSTOP)
- Experience required in:
- Care Coordination
- Discharge Planning
- DME/SNF/transport arrangements
- HMOs/IPAs
- Family meetings
- Epic experience required
- Travel experience preferred, but strong candidates can still be screened
- BLS required
- First-time travelers accepted with clinical approval
