Registered Nurse (RN) – Case Management
Location: Greer, SC 29651 | Work Setting: Hybrid
We are seeking a Registered Nurse (RN) with hospice experience to provide quality patient care and support case management and care coordination.
Responsibilities:
- Perform patient assessments and administer medications.
- Coordinate care, discharge planning, and follow-up services.
- Maintain accurate EHR/EMR documentation.
- Educate patients and families.
- Collaborate with interdisciplinary healthcare teams.
- Follow infection control and HIPAA standards.
Requirements:
- Active RN license
- 1 year hospice care experience required
- BLS, ACLS, or PALS certification
- EHR/EMR experience preferred
- Strong communication and clinical skills
