Looking for an experienced Epic Healthy Planet Analyst with:

✅ Epic Healthy Planet Certification – Required

✅ Healthy Planet build, registries & care gap logic

✅ Population Health / Care Management workflows

✅ Risk stratification & patient attribution

✅ HEDIS / eCQM / Quality Measures experience

✅ Epic Clarity/Caboodle & reporting experience preferred

✅ Compass Rose / SDOH / claims integration experience is a plus
