Leadership Role
Lancaster Nursing & Rehabilitation Center is seeking a Lead RNAC (Registered Nurse Assessment Coordinator / MDS Nurse) to join our clinical leadership team. This is a key role for an experienced MDS professional with strong PDPM, CMI, and Quality Measure (QM) expertise who can provide oversight, guidance, and clinical leadership within the MDS process.
This opportunity is ideal for a seasoned RNAC who enjoys mentoring, driving accuracy, and impacting both quality outcomes and reimbursement performance.
What We’re Looking For
Minimum 3 years of PDPM experience (required)
At least 2 years in a Lead RNAC / MDS Coordinator role
Advanced knowledge of MDS regulations, compliance, and audit readiness
Proven experience with CMI optimization and Quality Measures (QMs)
Strong organizational, communication, and leadership skills
Ability to collaborate effectively with interdisciplinary and clinical leadership teams
Lead RNAC / MDS Nurse Responsibilities
Provide leadership and oversight of the MDS process to ensure accuracy, timeliness, and compliance
Coordinate and manage all MDS assessments, care planning, and documentation per PDPM guidelines
Serve as a clinical resource and mentor for MDS staff and interdisciplinary team members
Partner with nursing leadership to support quality outcomes, survey readiness, and reimbursement accuracy
Monitor adherence to federal, state, and facility policies and regulations
Participate in performance improvement initiatives related to MDS, PDPM, and quality metrics
