LAMOUR Clinic and LAMOUR Community Health Institute were developed to provide an essential link between the community, the health care system, and the human services system. LOCATION:

Main offices in Randolph, Massachusetts and Braintree, Massachusetts. Administrative staff can be designated to work at any of our office sites within the regions we cover based on positions that are available and travel is required at times. This is an office position.

JOB SUMMARY:

The Intake Clinician acts as the lead clinician in conducting diagnostic assessments for enrollment into program services provided in Massachusetts. The clinician works with and serves as a documentation specialist for Utilization Review and assists in assigning direct care staff. The role maintains compliance with medical necessity criteria, record policies, and procedures through audits, collecting demographic information, and ensuring overall program compliance.

The Intake Clinician provides outpatient services in the office and in the community (homes, schools, and other community-based agencies). Duties include assisting with audits, compliance monitoring, quality assurance, and carrying a caseload. The role requires developing, implementing, and monitoring comprehensive quality assurance programming that streamlines new patient intakes into treatment services based on patient needs.

This position requires ongoing communication with staff to ensure medical necessity and the appropriate level of care. The clinician provides service coordination for assigned cases, including engaging parents and patients in the programs, conducting home visits, managing crises, maintaining collateral contacts, and coordinating transition planning. The clinician provides ongoing assessments every 90 days for assigned cases and participates in assessments and treatment planning, including completing written reports for enrollment into Applied Behavior Analysis (ABA), Substance Abuse and Recovery Support Services (RSS), Dialectical Behavior Therapy (DBT), In-Home Therapy (IHT), In-Home Behavioral (IHB), and other programs and services.

The role also includes conducting Utilization Review, initial intake assessments, and 90-day reassessments as needed for patients to maintain program services. Responsibilities include monitoring and following up with all clinicians on clinical documentation to ensure medical necessity across all programs. The Intake Clinician works with master’s- and doctorate-level clinicians as part of a multidisciplinary team that includes both professional and paraprofessional staff, providing coordination-of-care outreach and therapeutic support for behavioral health needs within clinic programs.

POSITION RESPONSIBILITIES AND TASKS:

  • Intake assessment, treatment plans, CANS medical necessity admission criteria, and enrollment packets are complete for the patient to be enrolled into program services
  • Must understand the medical necessity and be able to diagnose and assess patients participating in program services.
  • Must be able to complete assessment, CANS assessment, treatment plan, and progress note in accordance to a policy that meets the professional standards and timeframe and interdisciplinary and/or care review meetings for a plan of care
  • Completes all documentation needed and develops clearly defined interventions and measurable goals and outcomes that are consistent by a policy that meets the professional standards,
  • Conducts home visits to enroll patients into the program and for quality assurance compliance as needed.

  • Collaboration with Billing Department and obtain authorization for program services
  • Refer to other programs services when needed internal and external
  • Reviews all documentation for ongoing programs admission criteria and auditing
  • Collaboration with care coordinator via collard contact (via phone, attendance at IEP meetings, hospital discharge meetings, Care Planning Team (CPT) meetings for youth enrolled in ICC services)
  • Refers patients when needed to other programs Services for Psychological and Neuropsychological Testing, Medication Management, Substance Abuse program Services, In-Home Therapy Program (IHT)/ Family Stabilization Team ( FST), Community Support Program (CSP),Therapeutic Mentoring (TM), Dialectical Behavioral Therapy (DBT) Applied Behavioral Analysis (ABA).
  • Reviews all documentation of staff clinical direct care staff as part of supervision weekly complies measure of medical necessity documentation in medical record every 90 days and 180 days reviews, diagnose, assessment, treatment planning and services specification documentation, etc.
  • Collect and maintain supervision weekly documentation as part of quality assurance compliance
  • Conducts review with managed care insurance to obtain authorization for services.
  • Utilize comprehensive, home-based behavioral health assessment inclusive of the CANS, with fidelity regarding process notes, treatment plans and assessments and discharge plan
  • Conducts review with managed care insurance to obtain authorization for services
  • Will be trained in Applied Behavioral Analysis and Dialectical Behavior Therapy treatment methods and obtain licensure
  • Must have 32 minimum billable hours weekly, have a steady caseload and three clinic days
  • Progress Notes Match Reproductivity Reports submitted daily for as timesheet for billable hours
  • Completes Specialty Programs documentation progress notes within 24 hours and uploads inpatient file cabinet in EMR
  • Verifies patients after appointment or services in EMR.
