Responsibilities:
- Collaborate: Work closely with our interdisciplinary care team, including nurses, therapists, physicians, and other healthcare professionals to develop and implement individualized care plans.
- Coordinate: Collect and analyze medical information, submitting accurate and complete Minimum Data Set (MDS) assessments and care plans to Medicare and other regulatory agencies.
- Communicate: Effectively communicate with care team members, families, and healthcare providers to ensure seamless transitions and optimal care outcomes.
- Monitor: Monitor and report changes in resident conditions, providing timely and accurate information to the care team and families.
- Maintain: Maintain accurate and confidential records of resident care, assessments, and care plans.
