Lakeview is looking for you!!We are looking for a Community Integration Specialist in our Millpond Location in Westfield, WI . (Central Wisconsin)
Alternating weekends included.
To provide assistance and treatment to program participants at the facility. To help ensure active participation in the rehabilitation program. To participate as a member of the interdisciplinary treatment team. To provide education, assistance, supervision, safety, and behavior management for the program participant population.
Key Responsibilities:
- Functions at all times as a therapeutic agent by demonstrating teaching, reinforcing, anticipating, interacting, and noticing program participant behavior.
- Tends to the program participant immediate needs for safety and performs all aspects of participant care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Provides treatment in accordance with unit and facility policies through active participation with residents during activities, ADL's, school, work, meals, groups, etc., in a way that fosters independent functioning on the part of the resident.
- Displays age-appropriate interaction skills. Actively and professionally participates in exchange of information. Interacts with family members and others in a professional manner.
- Demonstrates consistent and reliable documentation on Education forms in accordance with Individual Service Plan (ISP) and prescribed treatment interventions.
- Documents changes and/or unusual behaviors/situations. Completes other unit paperwork (behavior plans, checklists etc.) as required.
- Demonstrates flexibility and responsibility in scheduling, coverage, and provision of clinical services to meet the needs of the residents and families.
- Participates activity in all assigned training programs to meet state and company requirements. Adheres to the administrative, clinical, and human resource policies of Lakeview Specialty Hospital & Rehab. Other duties as assigned.
Qualifications :
Skill, Knowledge, and Abilities: Knowledge of the sequela of brain injury and other neurological disorders. Knowledge of physical, cognitive, psychological, emotional, and/or communication disabilities. CBRF certifications preferred which include : Medication Administration, Fire Safety, First Aid/Choking and Standard Precautions. Individuals in this role should have knowledge and experience with basic care giving.
Education: Required: High school diploma or equivalent may include completion of certified nursing assistant course with certificate. Preferred: Degree in the area of Social Services. Certified as CNA if CNA.
Licensure: Valid driver’s license and good driving record preferred
Preferred at least one year of experience working within a residential based or behavioral program.
