Job Description
Provide physical therapy services to clients in their home
Work Schedule: 1.0 FTE; Monday-Friday 8-4:30, occasional weekends

Work Schedule: 1.0 FTE; Monday-Friday 8-4:30, occasional weekends
Required Qualifications
- Graduate of accredited physical therapy program
- Continuing education as required by state licensing boards
- MN or WI Physical Therapy (PT) License upon start date
- Must provide proof of MN and WI PT license within 4 months of start date, if both MN and WI PT licenses are not present on start date
- Must have a vehicle with auto insurance and a valid driver’s license and meet all criteria required under Lakeview’s Automobile Liability Guidelines
- Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
- Ability to read, write, and speak the English Language
