Social Worker Qualifications
Previous experience in a long-term care facility required in consulting
Oversee facility social worker
BS in Social Work
A love for people, especially the elderly
Excellent communication skills
Social Worker Responsibilities
Counseling residents and families.
Coordinating any necessary psyche services and behavior meetings.
Discharge planning, participating in care conferences and communicating effectively with the nursing team.
Keep accurate records for each resident you work with, including MDS logs, and report any changes in a resident’s condition to the nursing department immediately.
