Lakeside Special Care, a Generations Healthcare is in search of a Program Counselor to join our team.
Job Description
Assist with monthly written progress notes in the record written by members of the staff providing services, the notes shall be specific to the needs of each client and the program objectives and plans
Assist with reassessment and summary of the progress of the clients the
appropriateness of program objectives and the success of the plan
Assist with initial evaluation and assessment by facility staff of their medical, nursing, dietetic, social and physical needs within 15 days of admission unless evaluation has been completed by referring agency within 30 days prior to admission
Ensure individual clients admitted will had a psychological evaluation and
assessment by appropriate discipline within 45 days of admission
Ensure patients/clients rights, program requirements, treatment services and staff are maintained as required
Requirements:
Proof of COVID-19 Vaccination will be required for all facility employees as of September 30, 2021 (requests for accommodation due to disability or religious belief will be reviewed)
Shall have at least a Bachelor's degree or applicable experience
Must be able to read, write, speak, and understand the English language.
Must possess the ability to make independent decisions when circumstances warrant
such action.

Must possess leadership and supervisory ability and the willingness to work
harmoniously with and supervise other personnel.
Possess knowledge of direct resident care services which ensures the safety, comfort and protection of residents.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing practices.
Must be able to relate information concerning a resident's condition.
Must not pose a direct threat to the health and/or safety of other individuals in the workplace.
