JOB SUMMARY:
The Case Manages are responsible for the ongoing development of the Day Services programs to ensure associates receive current and appropriate services. This position is expected to communicate and cooperate with all pertinent staff regarding associate activities and progress. This position is responsible for carrying out all assigned day-to-day programs in such a way as to provide maximum benefits to the individual.
RESPONSIBILITIES:
- Instruct associates in vocational and developmental skills and prepare them to function as independent as possible
- Complete case management duties assuring that appropriate training goals are developed and that all staff members are routinely information or progress and needs
- Documentation, reports, updates and other communications are to be timely
- Instruct associates to always carry out assignments with safety in mind
- Develop behavior support plans and actively assist in the management of behaviors
- Demonstrate a public relations attitude in all communications with workers. Parents, caretakers and human service professionals
- Regularly check and case note on assigned associates
- Maintain confidentiality of associate information at all times
- Provide direct care and support as needed
- Train and lead assigned workers on production and other paid activities following established production procedures
- Demonstrate core values at all times
- Preform special assignments as needed
REQUIREMENTS:
- Respond quickly in the event of an emergency situation
- Apply CPI behavioral intervention techniques and serve as a member of the crisis team
- Drive agency vehicles as needed
- Initiate and follow up on injury/seizure reports as needed
- Demonstrate a friendly and positive attitude at all times
PHYSICAL REQUIREMENTS:
- Constant physical movement throughout the day
- Standing, walking, ending, reaching, pulling, pushing at all times
- Ability to lift 20 pounds
- Ability to verbally communicate and ability to hear conversation
CERTIFICATIONS AND LECENSES:
- Certification in CPI, First Aid, CPR, and AED
- Valid drivers license with a record free from serious or frequent violations
KNOWLEDGE, SKILLS AND ABILITIES
- High school diploma or comparable training
- Ability to read, write and communicate proficiently in English
- Ability to apply basic math skills to assigned work tasks
- Experience working with individuals with special needs
Job Type: Full-time
Pay: $16.00 - $19.00 per hour
