What are the responsibilities and job description for the CHF Clinic Coordinator position at Lakeside Heart and Vascular Center?
Position Overview:
The CHF and INR Clinic RN leads, plans, and manages specialized clinical programs to optimize care for heart failure patients. They bridge the gap between inpatient and outpatient care, ensuring accreditation compliance, quality improvement, and smooth patient transitions to reduce readmission rates.
Key Responsibilities:
- Perform acute clinical assessments, monitor vital signs, and manage patient triage calls.
- Provide comprehensive education on heart failure pathology, medication, dietary modifications, and symptom management.
- Program Administration & Quality Improvement
- Oversee and monitor adherence to specific regulatory and heart failure accreditation guidelines
- Represent the CHF program act as a primary point of contact for external agencies, and foster relationships between departments.
- Develop and adjust individualized care plans in direct collaboration with cardiologists and the interdisciplinary team.
- Conduct extensive medication reconciliation to ensure safety, consistency, and eliminate errors during transitions of care.
- Track patients post-discharge to ensure adherence to therapy regimens (e.g., diet, fluid restrictions, daily weights).
Qualifications:
- Education: Graduate of an accredited nursing program
- Licensure: Current, unrestricted RN license in the state of practice.
- Certifications: BLS and ACLS certification required.
Experience:
- Minimum 1 year of experience in critical care. Cath lab, interventional radiology, or similar setting preferred.
- Experience with moderate sedation and vascular access is highly desirable.
Skills:
- Strong clinical judgment and decision-making abilities.
- Excellent communication and patient education skills.
- Ability to thrive in a fast-paced environment and work well in a team setting.
Work Environment:
- Clinic setting.
- Daytime hours with no on-call requirements.
- Requires the ability to stand for long periods of time. Must be able to lift 50 pounds and move patients and equipment.
Pay: $35.00 - $45.00 per hour
