Are you a dedicated Registered Nurse with specialized wound care expertise and a passion for improving patient outcomes? Lakeside Health & Rehabilitation is seeking a Certified Wound Care Nurse (RN) to join our clinical team. In this vital role, you will oversee wound prevention, assessment, treatment, and education while collaborating with our interdisciplinary team to provide exceptional resident care.
What You'll Do:
- Perform comprehensive wound assessments and develop individualized treatment plans.
- Monitor and manage acute and chronic wounds, pressure injuries, surgical wounds, skin tears, and other complex skin conditions.
- Provide hands-on wound care treatments utilizing evidence-based practices.
- Collaborate with physicians, nursing staff, therapy, dietary, and other interdisciplinary team members to optimize healing outcomes.
- Conduct weekly wound rounds and maintain accurate documentation of treatments, progress, and outcomes.
- Educate nursing staff, residents, and families on wound prevention, treatment, and skin integrity management.
- Monitor infection prevention practices related to wound care and skin health.
- Ensure compliance with all state, federal, facility, and regulatory standards regarding wound care services.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to wound care outcomes.
- Current, unrestricted Virginia Registered Nurse (RN) license required.
- Current Wound Care Certification required (WCC, CWCN, CWON, WOCN, or equivalent).
- Previous wound care experience in a skilled nursing, rehabilitation, long-term care, or healthcare setting strongly preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent documentation and communication abilities.
- Knowledge of current wound care treatments, protocols, and regulatory requirements.
