Your Role:

As the Social Services Director / Admissions Coordinator, you will support residents emotionally and socially while coordinating smooth and welcoming admissions into our facility.

Responsibilities include:

• Manage and lead the Social Services Department

• Assess residents’ emotional, psychological, and social needs

• Develop and implement individualized care plans and interventions

• Coordinate and participate in care conferences and discharge planning

• Assist with admissions, referrals, and intake processes

• Build relationships with hospitals, referral sources, families, and community organizations

• Support residents and families through transitions and care planning

• Assist with MDS interviews and social service assessments

• Maintain accurate documentation and ensure compliance with regulations

• Collaborate with interdisciplinary teams to provide exceptional resident care

Who We’re Looking For:

• Two years of related experience preferred, ideally in long-term care or skilled nursing

• Social work or admissions experience preferred

• Strong communication, organizational, and interpersonal skills

• Detail-oriented and computer literate

• Dependable, compassionate, and team-focused

• Passionate about helping others and providing resident-centered care
