Lakeshore Talent is in search of a Licensed Practical Nurse (LPN) to join our client’s team in Lakewood, Colorado.
Position Details:
- Pay Range: $31.01 – $38.80 per hour
- Location: Lakewood, CO
- Travel: 75% travel required (must have personal vehicle, valid driver’s license, and insurance)
Requirements:
- Current State-Issued Licensed Practical Nurse (LPN) license
- First Aid and BLS certifications (American Heart Association or American Red Cross)
- High School Diploma or GED
- Personal transportation, valid driver’s license, and proof of auto insurance
Preferred Qualifications:
- 3 years of LPN experience in a clinic or long-term care setting
- Experience working with geriatric patients
- Knowledge of medications and treatments for older adults
Summary:
The In-Home Services LPN provides hands-on nursing care and supports participants in their homes and community settings. This position is responsible for delivering prescribed medical treatments, monitoring participant health, and ensuring compliance with care plans and regulatory standards
Key Responsibilities:
- Provide general nursing care, including administering medications and treatments according to care plans and medical orders
- Furnish services, treatments, and diagnostic or preventive procedures as assigned
- Perform phlebotomy functions and administer vaccinations and TB testing as needed
- Educate participants and family members on medications, treatments, and care plans
- Observe and document participant condition, reactions, and progress in clinical records
- Collaborate with physicians, RNs, and care team members to implement care plans
- Assist with reviewing and updating care plans based on participant needs
- Participate in quality management and utilization review activities
- Maintain accurate and timely electronic or written documentation
- Provide compassionate and professional care in participants’ homes or other designated settings
