What are the responsibilities and job description for the Cradle to Career and Supportive Housing-Dual Program Case Manager position at Lakeshore Community Action Program?
Lakeshore CAP is seeking a compassionate and motivated Dual Program Case Manager to support families with young children and individuals experiencing or at risk of homelessness. This position plays a key role in connecting community members to housing stability, family support services, and long-term success resources.
What You’ll Do
Cradle to Career Program:
- Conduct participant assessments and develop Individual Goal Plans
- Provide on-going case management to participating families
- Collaborate with interdisciplinary teams to address client needs
- Build partnerships with schools and community organizations
- Perform outreach to engage eligible families
- Maintain accurate client records and generate program reports
Supportive Housing Program:
- Complete intake and eligibility screenings
- Develop individualized service plans and monitor progress
- Provide case management and referrals to housing and support resources
- Enter and manage data in tracking systems
- Represent the program in community coalitions and partner meetings
- Participate in the semi-annual Point in Time Count

- Strong communication and organizational skills
- Comfortable using Microsoft Office and client data systems

- Reliable, dependable, and adaptable
- Ability to maintain confidentiality and professionalism
- Valid Wisconsin driver’s license, reliable vehicle, and insurance
Work Environment
- Primarily office-based with occasional outreach and community work
Physical Requirements
- Ability to sit or stand for extended periods
- Ability to lift up to 20 pounds occasionally
