This position is responsible for implementing and monitoring services as authorized by the Local Intellectual and Developmental Disability Authority (LIDDA) in accordance with each individual's

Person-Directed Plan (PDP)/individual Service Plan (ISP), behavior support plans, and HHSC certification principles.

In addition to direct service and care coordination responsibilities, the Lead Provider Care Coordinator provides clinical and operational leadership to provider staff by supporting consistent implementation of ISP/PDP objectives, modeling best practices, and assisting with

training, quality oversight, and problem resolution. This position supports individuals with

intellectual and developmental disabilities, including individuals with dual diagnoses, through

direct care, skills training, behavioral intervention, crisis response, and provider-level

coordination of services.

This role requires management of an intensive caseload and completion of extensive

HHSC-required documentation, including service delivery documentation, progress notes,

incident reports, and required waiver paperwork within established timelines. This position does

not perform LIDDA authorization, eligibility determination, or independent service planning, and

functions solely to ensure authorized services are delivered in compliance with HHSC standards.

QIDP Responsibilities (Provider Role)

  • Implement the Person-Directed Plan (PDP)/lndividual Service Plan (ISP) as authorized by the LIDDA.
  • Coordinate and oversee internal provider services to ensure authorized services are delivered as outlined in the ISP/PDP.
  • Monitor service delivery within the provider agency to ensure quality, consistency, and compliance.
  • Ensure direct care staff follow the ISP/PDP, behavior support plans, and applicable protocols.
  • Provide guidance, coaching, and support to staff regarding ISP implementation and service expectations.
  • Document service implementation, individual progress, and outcomes in accordance with HHSC and waiver documentation requirements.
  • Maintain compliance with:
  • HCS and TXHmL program rules & Billing Guidelines
  • HHSC Certification Principles
  • HHSC survey, monitoring, and audit requirements

QMHP Responsibilities (Provider Role)

  • Provide behavioral health and mental health services within scope of QMHP designation, including skills training, psychosocial support, and behavioral interventions.
  • Implement behavior support strategies using effective, least intrusive methods.
  • Provide crisis intervention and stabilization support as needed.
  • Document mental health services in accordance with HHSC behavioral health documentation standards.

Lead Role Responsibilities

  • Serve as a lead resource to provider staff for care coordination, service implementation, and documentation expectations.
  • Assist supervisors with onboarding, mentoring, and ongoing support of direct service staff.
  • Support consistency and quality across provider services and assist with addressing service delivery concerns.

Direct Care, Travel, and On-Call

  • Provide and/or oversee direct service to individuals served.
  • Provide transportation and travel frequently within the community, including between individuals' homes and provider service locations.
  • Maintain 24-hour responsibility for assigned individuals and participate in on-call rotation with stipend.

Salary: $21.79 - 24.06 Depending on experience

Minimum Qualifications

Must meet requirements for BOTH QIDP and QMHP

QIDP Qualifications

  • Bachelor's degree (or higher) in a human services-related field OR current licensure as a Registered Nurse (RN).
  • Minimum of one (1) year of documented experience working directly with individuals with intellectual or developmental disabilities.
  • Knowledge of IDD waiver services and ISP/PDP implementation.

QMHP Qualifications

  • Must meet Texas HHSC requirements to function as a Qualified Mental Health Professional (QMHP).
  • Education and experience sufficient to provide behavioral health interventions, skills training, and crisis services.

Additional Requirements

  • Ability to manage a high-volume, intensive caseload.
  • Strong leadership, organizational, and documentation skills.
  • Ability to complete extensive documentation accurately and timely.
  • Valid Texas driver's license and reliable transportation.
  • Ability to work independently and collaboratively in a team environment.

Preferred Qualifications

  • Master's degree in a human services or behavioral health field.
  • Experience providing services within HCS, TXHmL, or other Texas IDD waiver programs.
  • Previous experience in a lead, mentoring, or supervisory support role.
  • Experience working with individuals with dual diagnoses.
  • Knowledge of HHSC certification principles, incident management, and provider compliance requirements.
  • Bilingual (English/Spanish).
  • Experience with electronic documentation systems.
  • Experience with our electronic health record Oracle.
