Job Purpose 

The Per-Diem Speech Language Pathologist is responsible for implementing standards of care in speech-language pathology services, ensuring high-quality patient outcomes in the home health and hospice setting. 

 

Primary Duties and Responsibilities 

  • Conduct comprehensive evaluations and assessments of the patient’s home environment, identifying equipment needs related to speech and language functions.
  • Identify functional speech defects and develop a plan of care aimed at improving the patient’s speech and language abilities.
  • Administer speech therapy programs using specialized therapeutic techniques and/or equipment.
  • Recommend and facilitate the use of communication devices or aids as needed.
  • Collaborate with referring physicians and other healthcare team members to ensure coordinated, comprehensive care as part of the patient’s total plan of care.
  • Participate in case conferences, in-service training, and other meetings as required to maintain coordinated and comprehensive care.
  • Prepare and submit accurate and timely clinical notes in accordance with organization policy.
  • Maintain clinical competency in speech-language pathology practice and theory.
  • Identify additional patient and family/caregiver needs and make appropriate referrals to other services.
  • Instruct patients, family members, caregivers, and other healthcare personnel in the patient’s treatment plan as needed.
  • Supervise home health aides in procedures directly related to the speech-language therapy plan of care when applicable.
  • Perform other duties as required to facilitate the delivery of speech-language pathology services.

Note: The above statements are intended to represent the major duties and responsibilities of this role. Additional tasks may be required as needed. 

 

Qualifications 

  • Master’s or doctoral degree in Speech-Language Pathology (SLP).
  • Licensed as a Speech-Language Pathologist by the State or meets the requirements for unlicensed states, including:
    • Completion of 350 clock hours of supervised clinical practicum.
    • A minimum of 9 months of supervised full-time SLP services after obtaining the degree.
    • Successful completion of an approved national examination in SLP.
  • At least one (1) year of clinical experience in Speech-Language Pathology.
  • Strong verbal and written communication, and organizational skills.
  • Current CPR certification.
  • Licensed driver with a reliable vehicle, insured in accordance with organization requirements.

 

Working Conditions 

  • This is a traveling position requiring a reliable vehicle to navigate varied weather conditions within the geographic area.

 

Physical Requirements 

  • Prolonged walking or standing.
  • Ability to lift, position, or transfer patients and lift supplies and equipment.
  • Considerable reaching, stooping, bending, kneeling, or crouching.
  • Visual acuity and hearing necessary to perform required skills.
