PRIMARY PURPOSE:

The Case Management Team Lead ensures patients’ access to services and community resources through effective coordination and collaboration with staff within the agency as well as with community partners.

DESCRIPTION OF DUTIES & TASKS:

Under the direction of the Assistant Director, and within the center’s program policies and procedures, the Case Management Team Lead performs the following functions.

ESSENTIAL FUNCTIONS:

  • Provides oversight of the Adult Services program Case Management team including providing direct supervision of assigned case managers and monitoring the program’s delivery of monthly case management services.
  • Provides Case Management services to a small caseload as well as support their case management team in case of needing coverage or increased support to monitor higher acuity cases.
  • Provide individual supervision for all assigned staff. Educates and orients staff to program philosophy/values and goals to ensure understanding of, commitment to, and cohesiveness of program goals and vision.
  • Monitor each case manager’s caseload, provision of services and completion of documentation. Monitors staff morale and addresses the needs of staff performance as needed.
  • In conjunction with the Assistant Director, manage day-to-day case management concerns that arise via staff or patients.
  • Communicate and collaborate on treatment planning and care coordination amongst treatment team members to ensure patients are receiving quality care and are linked to needed resources to maintain independence in the community.
  • Document activities, service plans, and required metrics in the prescribed electronic health record, as identified by the agency in a timely manner.
  • Provides ongoing training and support with established staff as well as facilitating interviews and onboarding of new staff.
  • Has general knowledge of the program’s budget and monitors financial and statistical data to manage the program’s costs, revenues, and overall service delivery.
  • Participates in community-based organizations as a representative of Lakes Region Mental Health and participates on agency committees.
  • Provides Critical Time Intervention (CTI) services in line with the CTI fidelity
  • Performs other duties as assigned.

REQUIRED KNOWLEDGE, SKILLS & ABILITIES:

The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how the individual can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities. Must also possess the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.

  • Must be able to establish a rapport with the patients, and treat all patients with dignity and respect.
  • Ability to organize work-load so that his/her clinical and administrative duties are completed in a timely and thorough manner without the need for direct supervision.
  • Understands the LRMHC mission and applies this knowledge productively to shape internal policies and external contacts.
  • Must be capable of working closely with other agency staff to expand the knowledge of mental health services both across programs.
  • Strong verbal and written communication skills, and have the ability to read and analyze data and reports.
  • Must be able to tolerate periods of heavy workload, to include long working hours and the balancing of conflicting demands
  • Must be comfortable technology.
  • Attendance is a requirement of this position.

QUALIFICATIONS:

Education: Bachelor’s Degree in Social Work, Counseling, Psychology or related field

Experience: Supervisory experience. 2 to 3 years’ experience preferred.

License/Certifications: Valid driver’s license. Able to pass LRMHC criminal records and motor vehicles screening. Must maintain personal auto insurance at level commensurate with agency requirements.


Physical Requirements:

  • Is able to sustain light physical effort such as frequent standing, walking, bending, stooping, or reaching and/or occasional lifting of items 20 lbs. or more.
  • Is able to tolerate periods of heavy workload or excessive stress.
