Job Summary:

Assertive Community Treatment (ACT) is a patient centered, recovery oriented, mental health service delivery system. The ACT Community Case Manager provides community based therapeutic behavioral services for individuals with the most severe mental illness. The ACT CCM also plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services to assist individuals to integrate as independently and fully into the life of the community as possible. The ACT CCM works collaboratively as part of a multidisciplinary treatment team, and must demonstrate excellent clinical judgment, decision making and safety consciousness to assure safety and appropriate treatment for consumers in the community.

Essential Duties/Responsibilities:

  • Meet with patients/caregiver(s) (face to face) at intake, periodically and annually to obtain information to conduct a comprehensive assessment of needs.
  • Completes case management assessment and develops comprehensive care plans and individual treatment plans and monitors services received.
  • In collaboration with prescriber, Emergency Services, families and natural supports, provides ongoing assessment, monitoring, support and crisis interventions to patients as needed.
  • Assists patients in accessing and understanding budgeting, subsidized housing, food stamps, utilities and other related benefits.
  • Ongoing communication and coordination with patient/caregiver(s), guardians, landlords, and providers/collaterals, as needed.
  • Obtain a working understanding of state ACT fidelity requirements to ensure patients on the team are receiving quality care.
  • Review and monitor linkage to services identified in the Case Management Plan, link patients to services, including but not limited to: phone calls, scheduling appointments, verbal and written referrals to internal/external providers and the completion of associated documents.
  • Meet quarterly with patient and internal providers to obtain information to complete Quarterly Service Review documentation.
  • Complete all documentation and billing in compliance with standards set by state and agency policy.
  • Provide other duties as assigned by supervisor (medication prompts, etc.).
  • Attend meetings, trainings, and in-services as required and participate in regularly scheduled supervisions as required.
  • Attendance is an essential function of this position.

Required Skills/Abilities:

The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how the individual can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities. Must also poses the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.

  • Must be able to establish a rapport with the patients. Must treat all patients with dignity and respect.
  • Understands the LRMHC mission and applies this knowledge productively to shape internal policies and external contacts.
  • Must be capable of working closely with other agency staff to expand the knowledge of mental health services both within the Child and Family Team and across to other programs.
  • Must be able to develop strong working relationships with external services.
  • Must have basic computer skills, and strong verbal and written communication skills.
  • Must have the ability to read and analyze data and reports.
  • Must be able to tolerate periods of heavy workload, to include long working hours and the balancing of conflicting demands

Education and Experience:

  • Bachelor’s in mental health counseling, social work, psychology, nursing, counseling (preferred) or other related field with two (2) years of experience preferably in mental health. Meets qualifications to provide Functional Support Services as specified in He-M426-11 and He-M426-13.
  • Must have and maintain a valid NH driver’s license with current liability insurance which meets Lakes Region Mental Health Center requirements.

Work Schedule:

Schedule consists of mainly Monday-Friday with requirements to work 1 night a week and 1 weekend a month. On-call phone rotated through ACT staff once a month

Physical Requirements:

  • Is able to sustain light physical effort such as frequent standing, walking, bending, stooping, or reaching and/or occasional lifting of items 20 lbs. or more.
  • Is able to tolerate periods of heavy workload or excessive stress.

Pay: Up to $22.00 per hour
