Description
Lakes Region Community Services (LRCS) is accepting applications for a full-time (37.5 hours per week) Area Agency Coordinator. The Area Agency Coordinator supports intake, eligibility determination, and administrative processes for the area agency while ensuring accurate documentation, strong communication, and compliance.
Some of the essential job functions include but are not limited to:
- Serve as first point of contact for individuals and families inquiring about services; including occasional evening and weekend meetings as needed
- Process intake packet, including records from school, specialists, and guardians, to support eligibility determination for area agency services.
- Enter and maintain accurate individual data within the electronic medical record (EMR) or database system.
- Respond to phone calls and emails from individuals, families, or guardians; provide information and schedule appointments
- Manage NH Easy Registry process including reviewing the request, entering data into the registry, and communicating status updates.
- Oversee Pass Through process by collecting required documentation to ensure waiver compliance and complete Medicaid billing for reimbursement.
- Communicate with statewide service coordinators
- Associate degree with a major in business, human services, or related field and four years of experience or bachelor’s degree.
- Knowledge of Developmental Disabilities and Acquired Brain Disorder and familiarity with local and regional resources.
- Established connections within Belknap and Southern Grafton county communities preferred.
- Strong interpersonal skills and communication skills (written, phone, e-mail & in-person) with attention to detail.
- Ability to multitask, prioritize, and manage competing demands effectively.
- Proficiency in electronic spreadsheets, web based systems, and word processing software.
- Valid NH Driver’s license, reliable vehicle, and agency-determined minimal motor vehicle insurance coverage are required. Satisfactory completion of background and applicable pre-employment physical capacity screening checks are also required. We are an Equal Employment Opportunity Employer.
