Lakes Homes and Program Development, Inc. is seeking a Registered Nurse (RN) to also serve as the Designated Coordinator for our 245D-licensed Community Residential Services program in Mahnomen, MN.
The Mahnomen program is a community home serving four adults with intellectual disabilities. This job is ideal for someone who wants to utilize their nursing skills but prefers not to work in a traditional clinical setting. RESPONSIBILITIES
This program coordinator RN nursing position will oversee the medical needs of individuals who live in a community residential support program that is licensed under Minnesota department of human services statutes 245D.
As a nurse for Lakes Homes, you will complete in home medical assessments and guide unlicensed staff to follow through with nursing and physician orders. You will accompany individuals to appointments, complete medication/treatment orders and instruct staff in new medication or treatment procedures for individuals. You will work closely with medical providers and communicate with guardians and county case managers regarding health concerns. You will be responsible for maintaining current nursing notes on individuals.
You will oversee any medical cares and medication protocols in the homes. You will teach direct care staff, (DSP's) to be competent at giving medications and performing prescribed treatments. You will complete observed skills assessments to ensure that direct care staff demonstrate the ability to perform procedures consistent with Lakes Homes' policies and procedures, as well as those meeting requirements under your Minnesota nursing license; including but not limited to Blood pressures, home physical therapy, glucometer checks, ear care, nail care, peri care, AFO's TED socks, hearing aids, dentures and splints, reporting incidents and documenting narrative logs and Medication Administration Records. Competency documentation is completed and filed in personnel training files.
You will attend and participate in team meetings, aka care conferences and regular Lakes Homes' staff meetings.
You will be responsible for communicating with pharmacies regarding new medications or medication changes, as well as the routine filling of prescribed medications. You will work with medical supply providers for supplies and equipment needed for the day to day cares of the individuals who need them.
You will work with other nursing personnel at Lakes Homes, direct care staff, and managers. You will report directly to the executive administrator as your supervisor.
Your designated coordinator responsibilities are to develop, implement, and monitor individualized 245D support plans and health-related care. Ensure compliance with Minnesota DHS 245D regulations. Train and support direct care staff on person centered life styles for the individuals we serve. Maintain accurate, organized, and timely documentation. Collaborate with case managers, guardians, physicians, and other interdisciplinary providers. You will write support plans and assure they are being implemented effectively and in a timely manner. You will attend interdisciplinary meetings at least twice a year for each individual.
You will be expected to be flexible with your schedule, to meet the needs of the individuals we serve. Because we are a residential program, some evenings and weekends may be expected of you.

Ability to pass a Minnesota Netstudy criminal background study (Required)
BLS Certification (Preferred). RN Minnesota license (Required)
QUALIFICATIONS – REQUIRED
Current RN license in the State of Minnesota
Valid driver's license
Ability to pass a Minnesota DHS criminal background study
Strong communication, organization, and documentation skills
QUALIFICATIONS – PREFERRED
Knowledge of or experience with Minnesota DHS 245D licensing
Experience in disability services, community residential programs, or care coordination
-Experience with tilizing computer software such as Therap for documentation.
