Summary
We are seeking a Designated Coordinator to lead and support the delivery of individualized services for the people we serve. This role ensures services are provided according to each person’s Coordinated Services and Supports Plan (CSSP) Addendum, in full compliance with Minnesota’s 245D requirements.
Responsibilities:
- Develop and maintain person-specific CSSP Addendums and Individual Abuse Prevention Plans.
- Coordinate and monitor day-to-day service delivery across assigned locations.
- Supervise, coach, and support Direct Support Professionals to ensure quality service.
- Ensure proper documentation of services, incidents, and progress in accordance with 245D.
- Collaborate with families, guardians, case managers, and interdisciplinary teams.
- Provide or coordinate individualized staff training based on person-centered support plans.
- Maintain compliance with all licensing, health, and safety standards.
- Be available evenings and weekends as needed to support services and staff.
Qualifications (Per 245D Licensing Standards):
Candidates must meet one of the following:
- Associate’s degree in a human services field and 2 years of full-time direct care experience or
- Diploma or certificate in a human services field and 3 years of full-time direct care experience or
- 50 hours of education/training related to human services and 4 years of full-time direct care experience
Additional Requirements:
- Strong knowledge of 245D service standards and person-centered planning
- Excellent communication, organizational, and leadership skills
- Ability to prioritize and manage multiple responsibilities
- Valid driver’s license and acceptable driving record
- CPR/First Aid certification (or willingness to obtain)
