Summary:
The Integrated Care Coordinator (ICC) will support the Behavioral Health Team within a primary care clinic that utilizes an integrated care model. The ICC will work closely with the licensed behavioral health therapists and medical providers to ensure seamless care coordination for patients with behavioral health needs. This role involves patient advocacy, program navigation, and care coordination to optimize patient outcomes. The ICC is a key member of the care team, working to eliminate barriers to care and facilitating communication between patients, providers, and external resources. The ICC will also support the workflow development for integrated care services and assist with scheduling and care continuity.

* Standard Work: Integrated Care Coordinator
- Coordinate and manage patient appointments, including internal and external referrals, ensuring timely access to care.
- Participate in interdisciplinary team meetings to discuss patient needs and care plans.
- Coordinate care with external providers to ensure continuity and integration of behavioral health services into the patient's overall care plan.
- Anticipate patients' needs and deliver services in a timely, accurate, respectful, and compassionate manner.
- Provide patient education about behavioral health diagnoses and the integrated care model.
- Conduct brief assessments, including individual or family consultations as needed.
- Collect and document thorough social histories, assessments, and screening tools for care planning purposes.
- Implement evidence-based interventions tailored to the needs of adults, children, and families.
- Maintain documentation of referrals to external agencies and foster positive relationships with community partners.
- Ensure culturally competent services are available to meet the unique cultural needs of each patient and family member.

Education equivalent experience:

* Bachelor’s Degree in Social Work, Psychology, Human Services, or a related field.
Nonessential:
* Candidates who have completed their Master’s program in social work, counseling, or a related field and are seeking clinical supervision are strongly encouraged to apply.

Experience Preferred:
- One year or more experience working in a behavioral health setting.
- One year or more experience working on a integrated behavioral health team
- Experience working with Epic EHR
Certifications Essential: Must pass Level 2 FDLE background screening
