Position Responsibilities
Standard Work: Community Partnerships & Grants Coordinator
- Serve as the primary point of contact for designated community partners to support collaborative health programs, activities, and events.
- Coordinate scheduling, logistics, communication, and reporting for partnership-based wellness programs, health screenings, classes, and outreach events.
- Support program leads and instructors with materials, participant tracking, outreach, and day-of event operations.
- Monitor program data and performance outcomes to evaluate effectiveness and provide recommendations for improvement.
- Assist with the implementation and day-to-day coordination of all grant-funded programs across the Community Health Department (e.g., chronic disease programs, asthma initiatives, healthy lifestyle initiatives).
- Ensure adherence to grant requirements, timelines, deliverables, and reporting standards.
- Track budgets, expenditures, and performance metrics in collaboration with program staff and Finance.
- Research and identify potential new grant opportunities aligned with departmental priorities.
- Assist in writing, editing, and assembling grant proposals, letters of intent, and required documentation.
- Represent the department at community events, providing health information and resources.
Essential:
- Strong organizational skills with the ability to manage multiple projects and deadlines.
- Excellent written and verbal communication skills.
- Ability to build and maintain strong relationships with community partners.
- Proficient in Microsoft Office Suite and comfortable learning new systems for data tracking and reporting.
- Ability to work occasional evenings and weekends for events
Qualifications & Experience
- Bachelor Degree
- Bachelor’s degree in Public Health, Health Promotion, Social Services, Communications, or related field; or equivalent experience
Experience Essential:
- Minimum 1–3 years of experience in community health, program coordination, grant management, or related work.
- Experience working with community partners or nonprofit organizations.
Salary : $45,323 - $56,659
