Pay Rate: Min $45,323.20 Mid $56,659.20

Position Summary

The Community Health Partnerships & Grants Coordinator supports the planning, implementation, and evaluation of Community Health programs, with a special focus on managing and strengthening community partnerships and coordinating all grant-funded initiatives within the department. This role is responsible for guiding program activities, maintaining grant compliance, identifying new funding opportunities, and supporting community outreach efforts. The coordinator will work closely with internal teams, community partners, funders, and program participants to ensure seamless execution of programs that improve the health and well-being of the communities served by Lakeland Regional Health.

Position Responsibilities

Standard Work: Community Partnerships & Grants Coordinator

  • Serve as the primary point of contact for designated community partners to support collaborative health programs, activities, and events.
  • Coordinate scheduling, logistics, communication, and reporting for partnership-based wellness programs, health screenings, classes, and outreach events.
  • Support program leads and instructors with materials, participant tracking, outreach, and day-of event operations.
  • Monitor program data and performance outcomes to evaluate effectiveness and provide recommendations for improvement.
  • Assist with the implementation and day-to-day coordination of all grant-funded programs across the Community Health Department (e.g., chronic disease programs, asthma initiatives, healthy lifestyle initiatives).
  • Ensure adherence to grant requirements, timelines, deliverables, and reporting standards.
  • Track budgets, expenditures, and performance metrics in collaboration with program staff and Finance.
  • Research and identify potential new grant opportunities aligned with departmental priorities.
  • Assist in writing, editing, and assembling grant proposals, letters of intent, and required documentation.
  • Represent the department at community events, providing health information and resources.

Competencies & Skills

Essential:

  • Strong organizational skills with the ability to manage multiple projects and deadlines.
  • Excellent written and verbal communication skills.
  • Ability to build and maintain strong relationships with community partners.
  • Proficient in Microsoft Office Suite and comfortable learning new systems for data tracking and reporting.
  • Ability to work occasional evenings and weekends for events

Essential:

Qualifications & Experience

  • Bachelor Degree

Essential:

  • Bachelor’s degree in Public Health, Health Promotion, Social Services, Communications, or related field; or equivalent experience

Other Information:

Experience Essential:

  • Minimum 1–3 years of experience in community health, program coordination, grant management, or related work.
  • Experience working with community partners or nonprofit organizations.

Salary : $45,323 - $56,659
