What are the responsibilities and job description for the Nurse Liaison - Admissions Nurse Liaison - Admissions position at Lake Taylor Transitional Care Hospital?
The Nurse Liaison works in coordination with the admissions team including the appropriate physicians to: coordinate efforts within the admissions office to ensure timely response to acute care hospital referrals, screen patients for appropriateness of admission to inpatient rehabilitation, communicate with the admitting and consulting physicians, effectively speak with patients and family members throughout the evaluation process, as appropriate, facilitate admissions to Lake Taylor’s Rehabilitation department including required pre-authorization and pre-admission documentation, work with all health carepartners toward the goal of problem solving and facilitating referrals to the appropriate level of rehabilitation.
EDUCATION: B.S.N. Required. CONSIDERATION WILL BE GIVEN TO AN APPROPRIATE COMBINATION OF EDUCATION/TRAINING AND EXPERIENCE
EXPERIENCE: 3-4 years Professional clinical experience Required
1-2 years previous skilled nursing and acute care highly desirable Required
