Description Of Duties/Essential Functions

ASSIGNED TO A RESIDENTIAL UNIT, DEVELOPS, IMPLEMENTS AND EVALUATES INDIVIDUAL HABILITATION PLANS FOR PERSONS WITH DEVELOPMENTAL DISABILITIES IN A CASE LOAD. CO-CHAIRS INTERDISCIPLINARY TEAM MEETINGS. INTEGRATES CENTER AND COMMUNITY-BASED SERVICES AS PART OF THE SERVICE PLAN. ENSURES TRAINING IS PROVIDED. MONITORS AND EVALUATES PROGRAMS; COMMUNICATES PROFESSIONALLY WITH PARENTS, GUARDIANS, SCHOOLS AND OTHER SERVICE PROVIDERS. CONDUCTS IN-SERVICE TRAINING FOR STAFF.

Minimum Requirements

MUST MEET MINIMUM REQUIREMENTS FOR QUALIFIED MENTAL RETARDATION PROFESSIONAL(QMRP). REQUIRES BACHELOR'S DEGREE IN HUMAN SERVICES FIELD AND ONE YEAR EXPERIENCE WORKING WITH PEOPLE WITH DEVELOPMENTAL DISABILTIES.

Work Hours & Location/Agency Contact

8:00 A.M. - 5:00 P.M.,

TWO LATE DAYS 12:30 P.M. - 9:30 P.M.

TWO EARLY DAYS PER MONTH - 6:00 A.M. - 2:30 P.M.

PLUS ONE SATURDAY OR ONE SUNDAY PER MONTH.
