Schedule: Full-Time | Monday - Friday | 8:00 AM - 4:30 PM

We are seeking a compassionate and dedicated Licensed Social Worker to join our team and play a key role in enhancing the quality of life for those we serve.
Position Summary:
The Licensed Social Worker will provide emotional support, discharge planning, care coordination, and advocacy for residents and their families. This role is essential in ensuring the social, emotional, and psychological needs of our residents are met in a caring and respectful environment.
Key Responsibilities:
- Conduct psychosocial assessments and develop individualized care plans
- Provide counseling and support to residents and their families
- Assist with discharge planning and community resource coordination
- Advocate for resident rights and promote quality of life
- Collaborate with the interdisciplinary care team to address resident needs
- Maintain accurate and timely documentation in compliance with regulatory standards
Requirements:
- Bachelor's degree in Social Work (BSW) required
- Current state licensure as a Social Worker
- Experience in a skilled nursing or long-term care setting preferred
- Strong interpersonal, organizational, and communication skills
