Position Summary
The Certified Nurse Aide (CNA) - Rehab assists nursing personnel in providing direct and indirect patient care to rehabilitation patients under the supervision of a Registered Nurse. The CNA supports activities of daily living, promotes patient comfort and safety, and contributes to the overall rehabilitation process while maintaining compassionate, patient-centered care.

The CNA supports activities of daily living, promotes patient comfort and safety, and contributes to the overall rehabilitation process while maintaining compassionate, patient-centered care.
Essential Functions
- Assists patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility.
- Obtains and records vital signs, weights, intake and output, and other patient care data as directed.
- Assists with ambulation, transfers, positioning, and range-of-motion exercises.
- Observes and reports changes in patient condition, behaviors, or concerns to the licensed nurse promptly.
- Maintains a safe, clean, and organized patient environment.
- Answers call lights and responds promptly to patient needs.
- Assists with patient admissions, discharges, and transfers.
- Provides emotional support and encouragement to patients during their rehabilitation journey.
- Assists with transporting patients to therapy sessions, diagnostic procedures, and other departments as needed.
- Ensures compliance with infection prevention, patient safety, and hospital policies and procedures.
- Documents care provided accurately and timely in accordance with hospital standards.
- Collaborates with nurses, therapists, physicians, and other healthcare team members to support positive patient outcomes.

Minimum Education
- High School Diploma or GED preferred.
- Current Kentucky Nurse Aide Certification required.
- Current Basic Life Support (BLS) certification required within established timeframe.
- Ability to provide compassionate, patient-centered care.
- Effective communication and interpersonal skills.
- Ability to prioritize and manage multiple tasks in a fast-paced environment.
- Strong observation and documentation skills.
- Basic computer and electronic medical record proficiency.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Previous experience as a Certified Nurse Aide in an acute care hospital, rehabilitation, long-term care, or skilled nursing environment preferred.
- Experience caring for rehabilitation patients preferred.
