What are the responsibilities and job description for the Staff Nurse (part-time) position at Lake Country Area Agency on Aging?
General Statement of Duties:
Under the direction of the President/CEO and in collaboration with the PT Registered Nurse, is responsible for the supervision of the staff of the Personal Care department and the Nursing functions of the Hazelwood House Adult Day Health Care Center. She/he will perform all of the duties of a personal care nurse according to state and federal guidelines regulating Medicaid Personal Care and Adult Day Care. She/he will develop the appropriate plan of care for clients and monitor it and related staff. She/he shall also work closely with schedulers and perform evaluations on employees.
Specific duties for Personal Care and Adult Day Care:
1. Monitor aides.
2. Provide leadership, direction and evaluation of aides.
3. Develop personal care plans for clients that aides will use with client.
4. Visit each personal care client every thirty days or, as required by state guidelines, to review care plan and assess the condition of the client.
5. Assist in maintaining all records that pertain to programs within the responsibility of Medicaid Personal Care, Hazelwood House ADHC, IIIB, CSP and Private Pay Personal Care.
6. Be available by telephone during client service hours, PRN.
7. Act as an advocate for clients needing services not provided by LCAAA.
8. Attend meetings and educational functions as needed.
9. Work closely with billing supervisor to assure that aide records are obtained so that billing can take place according to schedule.
10. Assist the President/CEO with monitoring and expansion of the programs.
11. Participate in In Home Care Departmental meetings.
12. Participate in Quality Assurance reviews as required by licensing authorities.
13. Participate in orientation of new aides, in-service training, and continuing education programs.
14. Complete initial admissions for ADHC and Personal Care recipients.
15. Administer and monitor the health needs of the ADHC recipients.
16. Maintain a presence of a minimum of 8 hours each month at the ADHC.
17. Plan, organize and update Plan of Care for ADHC recipients every 6 months and the DMAS 99 for ADHC recipients every 90 days unless conditions change to warrant more frequency.
18. Attend Interdisciplinary meetings as required by policy.
19. Provide instruction and direct supervision to ADHC Aides as needed for staff.
20. Coordinate with ADHC Coordinator to provide continual training.
Required Knowledge and Abilities:
Administrative knowledge and experience. Skills in program planning, personnel management, public relations, ability to work with the elderly and excellent skills in written and verbal communications especially with staff. Must possess and maintain a valid Driver's License. Must maintain a driving record of "0" points or better and be insurable under the agency's Certificate of Insurance for vehicle coverage.
Acceptable Experience and Training:
Must be a registered nurse, have two years of personal care experience and be currently registered by the Commonwealth of Virginia and the State Board of Nursing. This position calls for administrative and confidential work of high responsibility. Employees of this class are responsible for the overall implementation of assigned and special tasks, coordination and planning, to include supervision of subordinate employees.
