Responsibilities of the Director of Social Services:
- Plan and implement a comprehensive social services program, offering counseling and support services for residents and their families.
- Evaluate and understand residents' medical-related social needs, formulating appropriate treatment plans within their total care plan.
- Coordinate and conduct family meetings, fostering ongoing discussions between residents, their families, and the treatment team.
- Maintain residents' rights while acting as a liaison between all involved parties.
- Coordinate discharge planning, assisting in the creation of organized discharge plans for all residents.
- Ensure residents and their families receive the highest quality service in a compassionate atmosphere that recognizes individual needs and rights.

Requirements:
- Bachelor’s Degree or higher, preferably in Social Work.
- Previous experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF) settings.
