Job Summary
Geriatric Mental Health Therapist
The position is located at Sarah Bush Lincoln Fayette County Hospital, and is managed by Lake Behavioral Health. The program serves those aged 65 and older experiencing mental health issues like depression and anxiety associated with life changes.
The Program Therapist plays a vital role in patient care services, focusing primarily on the emotional, behavioral, and mental well-being of the patients. Under the guidance and supervision of the Medical Director/Assistant Medical Director, the Program Therapist will perform assessments and provide individual, group, and family therapy. Working collaboratively with the Treatment Team, the Therapist will develop and update the treatment plans as necessary. Additionally, the Program Therapist will act as a Case Manager to coordinate discharge planning, ensuring effective communication with caregivers and family members, while maintaining connections with referral sources and other service providers.
Duties and Responsibilities
- Actively support the mission, philosophy, and goals of Lake Behavioral Health and the host hospital.
- Utilize appropriate techniques for evaluating and treating patients.
- Interview patients, families, and caregivers to gather history and develop the treatment plan.
- Provide individual, group, and family therapy as directed by the Medical Director or Assistant Medical Director.
- Communicate with patients and caregivers to identify treatment goals and progress.
- Consult with other disciplines regarding treatment plans, patient needs, and referrals.
- Maintain regular contact with facilities, referral sources, agencies, and other professionals involved with the patient.
- Participate actively in treatment team meetings, treatment plan development, and necessary revisions.
- Develop and implement appropriate discharge planning with patients, caregivers, agencies, and/or facilities.
- Document all clinical and case management services in compliance with CMS and other accreditation standards.
- Assist with all paperwork requirements from pre-admission to post-discharge.
- Comply with all administrative, clinical, human resources, regulatory agencies, state and local regulations, and host hospital standards.
- Adhere to work practices aligned with Lake Behavioral Health and the host hospital mission, medical staff bylaws, and departmental and human resources policies.
- Maintain the highest ethical standards in all program activities.
- Assist in referral development and engage in networking opportunities.
- Participate in and comply with the program’s performance improvement and quality measures.
- Fulfill other duties as assigned.
Qualifications
- Master’s degree in social work, psychology, counseling, or a comparable behavioral healthcare degree.
- Must be licensed to practice independently in the respective discipline & state. Consideration may be given to candidates requiring supervision.
- Experience with the program’s population and in a behavioral healthcare facility is preferred.
Clinical supervision for licensure is provided.
