What are the responsibilities and job description for the LPN (Licensed Practical Nurse) position at Lafayette Internal Medicine Clinic?
Our internal medicine practice is seeking experienced LPN applicants. This position is within our Chronic Care Management Department. The ideal candidate would have a passion for patient care and enjoy working in a team centered environment. Our mission is to deliver high quality and compassionate care to all of our patients.
Primary Job Duties and Responsibilities:
- Chronic Care Management
- Implementation and review of care plans for an assigned group of patients
- Case management services: including thorough patient triage and clear communication with the treating provider(s)
- Contacting patients via phone or patient portal regarding test results once reviewed by the provider
- Prior Authorizations and Pre-Certs
- Follow up on referrals and any scheduled testing with outside facilities as ordered by the provider
- Population health: ER follow up, transition of care services, and preventative health initiatives.
- Assist with hands-on patient care as needed
- Medication and vaccine administration as needed
Job Requirements:
- Current LPN License
- BLS Certification
- Electronic Health Record (EHR) experience preferred
