MDS Coordinator

We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments for residents in skilled nursing facilities, ensuring compliance with federal and state regulations. Your expertise will support accurate clinical documentation, optimize patient care planning, and facilitate seamless communication across multidisciplinary teams. The ideal candidate is energetic, highly organized, and passionate about improving patient outcomes through precise data management and clinical review.

Responsibilities

• Conduct comprehensive MDS assessments in accordance with CMS (Centers for Medicare & Medicaid Services) guidelines, ensuring timely completion for each resident.
• Collaborate with nursing staff, physicians, social workers, and other healthcare professionals to gather accurate clinical information for documentation review.
• Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to input and verify data, maintaining up-to-date and compliant medical records.
• Perform utilization review and management activities to support appropriate care levels, including discharge planning and hospice care coordination.
• Ensure coding accuracy by applying ICD-10 (International Classification of Diseases), CPT (Current Procedural Terminology), and DRG (Diagnosis-Related Group) coding standards to support billing and reimbursement processes.
• Maintain compliance with HIPAA regulations by safeguarding patient information during all documentation review activities.
• Stay current on evolving NCQA (National Committee for Quality Assurance) standards and CMS requirements to uphold high-quality clinical documentation improvement practices.

Requirements

• Proven experience with MDS coordination within a skilled nursing or long-term care setting.
• Strong knowledge of ICD-10, ICD-9, CPT coding, DRGs, and Medicare regulations related to inpatient and outpatient care.
• Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
• Critical care experience or ICU background is highly desirable; inpatient or hospital experience preferred.
• Knowledge of physiology, anatomy, medical terminology, and medical records management.
• Experience with utilization review, case management, discharge planning, and health information management.
• Ability to navigate complex documentation review processes while maintaining attention to detail and compliance standards.
• Excellent communication skills to collaborate effectively across multidisciplinary teams in fast-paced environments such as Level I or Level II trauma centers or pediatric units.
• Certifications related to nursing or health information management are a plus; a background in managed care or home care is advantageous.
