Job Overview
LA REMEDY Home Health is currently seeking a Clinical RN Case Manager to join our team in Inglewood. This is an in-office leadership role responsible for clinical oversight, coordination of patient cases, and ensuring regulatory compliance for our home health services.
Position Details
Location: Inglewood (In-office)
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Pay Range: $55 – $65 per hour ($114,400 – $135,200 annually)
Benefits: Paid time off, 50% health insurance matching, and 401k
Responsibilities
- Establish and monitor individualized Plans of Care (POC) in collaboration with the interdisciplinary team.
- Provide clinical oversight for skilled nursing visits, including wound care, IV therapy, and medication administration.
- Support field nurses with clinical guidance and troubleshooting.
- Ensure all documentation (OASIS, assessments, and progress notes) is accurate and compliant.
- Coordinate new referrals, recertifications, and discharges within Data Soft Logic (DSL).
QUALIFICATIONS
- Active, unrestricted California Registered Nurse (RN) license.
- 3–5 years of clinical nursing experience as a Case Manager in a home health setting.
- In-depth knowledge of OASIS, CMS, and home health regulations.
- Strong leadership, communication, and organizational skills.
