Position Function Summary
Under the direction of the Behavioral Health Director, the Behavioral Health Case Manager will work independently to provide coordination of care and services to patients in order to assist them to overcome the problems and needs associated with emotional and mental conditions.

Essential Responsibilities
  • Schedules patients and conducts reminder calls;
  • Provides targeted case management services to Behavioral Health patients, with special attention and understanding of the needs and circumstances related to emotional and mental health;
  • Carries out Case Management within the context of on-going needs, care and/or treatment planning, and monitoring.
  • Help patients with life skills.
  • Assists with developing, implementing, and improving service delivery frameworks, quality assurance and improvement of measurement tools, and data compilation and interpretation for reporting purposes.
  • Uses preventative actions to manage needs before a crisis develops, especially patient psychiatric needs that impact part-time psychiatrists.
  • Coordinates care and needs with behavioral, psychiatric, and medical providers to facilitate case coordination and appropriate information sharing, and assists with coordination of patient support services to meet the basic needs of individuals by:
    • Assisting behavioral health patients with Medicaid and Medicare referrals to on-site application assistance staff.
    • Assisting with access to mental health and substance treatment services.
    • Providing crisis assistance.
    • Providing direct assistance to psychiatry staff with patient logistical needs, including appointments, scheduling, patient rooming, medication and prescription follow up and troubleshooting, and general problem solving.
    • Offers a management perspective of cases and systems as related to BH department.
  • Works in conjunction with outside agencies and service providers to facilitate case coordination and appropriate information sharing.
  • Intercedes on behalf of patients to ensure equity and appropriate services, especially with elderly and special needs patients.
  • Assists patients in crisis to stabilize through direct interventions and the Dr. Green protocol and mobilizing needed supports and services.
  • Practices safety protocols and de-escalation techniques at all times.
  • Sets and adheres to boundaries with patients.
  • Ensures safety by informing the Behavioral Health Director and/or appropriate office staff of Case Manager’s location during outreach and other patient centered activities, including while coordinating with medical providers.
  • Networks with and maintains relationships with outside agencies and organizations to increase awareness, competence, and resources pertaining to the behavioral health needs.
  • Provides weekly and monthly reports, as requested by Behavioral Health Director.
  • Maintains appropriate patient records for continuity of care, treatment planning, and patient tracking.
  • Verifies insurance eligibility upon appointments being made for all BH providers.
  • Accepts bankcards over the phone for payment of fees or cost of goods/services rendered according to established procedures.
  • Performs responsibilities as assigned.

Job Requirements

Education, Experience and Skills/Training
  • Bachelor’s degree in social services, Counseling, Social Work and two years of experience in a related field. In lieu of a degree, the incumbent must possess sufficient prior work experience in progressively more responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.
  • Certified Community Health Worker, preferred.
  • Ability to work and manage multiple priorities in a fast-paced environment to meet established goals and deadlines, and excellent communications skills, verbal, written, and listening;
  • Ability to work with the mentally ill, substance abusers, and persons who have co-occurring medical and mental health and/or co-morbid disorders.
  • Ability to demonstrate sensitivity to the multicultural nature of the service population.
  • Knowledge of community resources.
  • CPR/BLS Certification, required.
  • Fluent in both English and Spanish (bilingual), required;
