As an LPN you will play a crucial role in providing high-quality healthcare services to our diverse patient population. You will work collaboratively with a multidisciplinary team of healthcare professionals to deliver patient-centered care and promote the overall wellness of our community.
Key Responsibilities
- Administer medications and treatments in accordance with provider orders and nursing standards.
- Assist with patient education, ensuring they understand their treatment plans and are empowered to manage their health effectively.
- Perform nursing procedures such as wound care, injections, and point of care testing.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Relay to patients important information regarding their health and current treatment plans.
- Maintain accurate patient records and ensure compliance with regulatory standards.
- Current Oregon LPN licensure.
- CPR certification.
- Strong communication and interpersonal skills.
- Spanish fluency is preferred
- Compassionate and patient-centered approach to care.
- Ability to work effectively in a team-based healthcare environment.
- Prior experience in a community health or primary care setting is a plus.
