Job Summary
The Service Facilitator’s position is a Part-Time 1099 Independent Contractor Position. This position description is mainly to provide ongoing supervision of Individuals with disabilities receiving services through their insurance or a Medicaid Waiver from (DMAS) the Department of Medical Assistance Services in Virginia. The position requirements prefer that the Service Facilitator/Independent Contractor, possess a minimum of an undergraduate degree in a human services field or be a RN currently licensed to practice in Virginia. It is suggested, that the Service Facilitator also have at least two years of satisfactory experience in the human services field working with persons with severe disabilities or the elderly. The SF will be responsible for conducting monthly and quarterly routine in-person visits to those receiving services and care and providing written documentation via required reports on those services as required by Medicaid and their respective insurance companies. Note pay for this position is PER VISIT not PER HOUR and you must live in the State of Va to be considered.
