Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and dignity.
- Monitor patients’ health by taking vital signs, observing for changes, and reporting findings accurately while adhering to HIPAA regulations.
- Administer medications and follow care plans meticulously to support ongoing health needs.
- Prepare nutritious meals tailored to dietary requirements and assist with cooking and meal planning.
- Support residents with mobility using equipment like Hoyer lifts or assisting with heavy lifting to prevent injury.
- Maintain a clean, safe environment through routine cleaning tasks and proper infection control practices.
- Document care activities thoroughly, including patient observations, medication administration, and incident reports to ensure compliance and continuity of care.
Skills
- Proven experience in home health, assisted living, nursing homes, or group homes with a focus on senior care.
- Knowledge of patient observation techniques, dementia care (including Alzheimer’s and memory care), diabetes, and mental health.
- Required certifications for CPR, mental health, and dementia specialty trainings.
- Strong communication skills for collaborating with healthcare teams, social workers, families, and residents.
- Physical stamina for performing tasks that involve heavy lifting or assisting with mobility devices like Hoyer lifts.
- Experience working within care plans and supporting patients’ emotional well-being through compassionate direct support.
