Care Manager RN — Remote (Michigan)
Contract: 12 month contract with possible extension
Work Arrangement: 100% remote
License Requirement: Michigan State RN License
1. Mandatory Requirements to Apply
- Nursing Diploma or Associate Degree in Nursing (required)
- Bachelor’s Degree in Nursing (strongly preferred)
- 5 years clinical nursing experience (acute, post-acute, or community)
- 2 year case management experience in a managed care setting (preferred)
- Experience managing patients via phone, digital platforms, or telehealth (preferred)
- Intermediate computer proficiency (Microsoft Excel, Outlook, Teams, Word, etc.)
- Ability to work independently and collaboratively within multidisciplinary teams
2. Roles & Responsibilities
- Lead coordination of a multidisciplinary care team (social workers, dietitians, pharmacists, physicians, clinical support)
- Conduct comprehensive patient assessments and develop individualized care plans
- Engage members/caregivers to identify gaps, address barriers, and improve overall health outcomes
- Monitor member progress, update care plans, and maintain accurate documentation
- Provide education on health literacy, medication adherence, nutrition, and self-management
- Support smooth transitions of care between facilities and home to ensure continuity
3. Pay and Benefits
- $35-39 per hour
- 9 days of Paid Time Off
- $500 sign on bonus in form of gift card upon completion of 3 month with Kyyba/Client

Pay: $35.00 - $39.00 per hour
