We are expanding our team to include a dedicated Targeted Case Manager to enhance care coordination and empower families on their journey.
Position Overview:
We are seeking a passionate and organized Targeted Case Manager to support patients and families by helping them access medical, educational, therapeutic, and community resources. This position is ideal for someone who wants to make a meaningful impact and grow with our innovative clinic.
Key Responsibilities:
- Conduct comprehensive assessments to determine patient and family needs
- Develop and maintain individualized case management plans
- Coordinate services across medical, behavioral, educational, and community systems
- Assist families with navigating Medicaid waivers, school-based services (IEPs/504s), and therapy providers
- Monitor progress and adjust plans as needed
- Maintain accurate documentation in compliance with clinic protocols and payer requirements
- Collaborate closely with clinicians and support staff to ensure holistic care
- Provide education and advocacy support to families
Qualifications:
- Bachelor’s degree required in Social Work, Psychology, Education, Human Services, or a related field
- Targeted Case Management certification or willingness to complete required training upon hire
- Experience working with individuals with autism or developmental disabilities is highly recommended
- Strong knowledge of community resources, especially in Kentucky (or willingness to learn)
- Excellent communication, documentation, and organizational skills
- Empathy, cultural sensitivity, and a collaborative mindset
- Tech-savvy and able to work in electronic health records and cloud-based systems
Position Details:
- Contract Type: 1099 Independent Contractor
- Part-Time Hours: Flexible scheduling; some remote work may be possible
- Pay: Competitive hourly rate based on experience and credentials
- Growth Opportunity: This role may evolve into a permanent staff position as the clinic expands
