Mode of Work- onsite
Mode of hire- Onsite
Shift Time: Flexible Schedule; Tuesday-Saturday or Monday-Friday. 7:30AM-15:30PM, 8:00AM-16:00PM, 9:00AM-17:00PM or 10:00AM-18:00PM.
Intermediate Level - The resource is able to successfully complete tasks as requested. Help from an expert may be required from time to time, but the resource can usually perform the skill independently.
Minimum Qualifications:
ò Bachelor of Social Work from an accredited Social Work program, or equivalent education and experience.
ò Basic computer skills, including proficiency in Microsoft Office products.
Role Description:
This position provides support services to ensure quality care for consumers receiving long-term living services. Responsibilities include:
ò Conducting on-site monitoring reviews of service providers to verify compliance with State and Federal guidelines.
ò Reviewing written documentation and meeting with consumers and stakeholders to evaluate service quality.
ò Monitoring issues related to consumersÆ medical and financial eligibility.
ò Statewide travel is required, including occasional overnight stays.
Pay: Up to $27.00 per hour
